What CQC reported

On 24 September 2026, CQC reported that Brantwood Hall Care Home in Wakefield remained rated inadequate following a July inspection. Inspectors found continuing breaches relating to staffing and management, alongside further breaches concerning person-centred care and safe care and treatment.

The findings included gaps in dementia training and skin-integrity records that were not accurate or detailed enough. CQC said it had begun further regulatory action, which the provider could appeal.

Questions for your next care records review

ServPatch’s practical takeaway is to review whether records help staff act on a person’s changing needs. For a small sample of recent concerns, ask:

  • Does the record describe the observation clearly enough for the next person providing care?
  • Is the agreed action assigned to someone, with a review point?
  • Can the manager see whether follow-up actually happened?
  • Do training reviews address the needs of the people currently supported?

Make follow-up visible

At handover, review open concerns alongside completed tasks. Recording an issue is only the first step; the team also needs to understand what happened next. These are suggested review questions, not a replacement for CQC’s requirements.

Use our care records checklist for CQC readiness to structure a wider review.
