Use this page to request deletion of your ServPatch account or personal data. You can email us directly or complete the request form below.

How to request deletion

  1. Use the email address linked to your ServPatch account.
  2. Include the name of your care-provider organisation.
  3. Tell us whether you want your account, your personal data, or both deleted.
  4. Send the request to [email protected], or use the form on this page.

Please do not include resident or service-user health information in your request.

How ServPatch accounts work

Your ServPatch login is created by an authorised administrator at your care-provider organisation using the ServPatch dashboard. Your login ID is then sent to you by email.

The ServPatch app lets you sign in and reset your password. It does not provide public self-service account registration. To close an account or request data deletion, follow the steps on this page.

What happens after you make a request

We will verify your identity and determine whether ServPatch or your care-provider organisation is responsible for handling the request.

Information ServPatch may delete

Where ServPatch controls the information, we may delete or anonymise:

  • Account profile information
  • Contact details linked to your account
  • Active login sessions and authentication access
  • Other personal data that is no longer required

Care records controlled by your organisation

Care records entered by a care provider are controlled by that organisation. Requests concerning resident or service-user records should normally be submitted to the relevant care provider. ServPatch will assist the provider in completing verified requests.

Information that may be retained

Certain information may be retained where required for safeguarding, regulatory compliance, security, fraud prevention, contractual obligations or applicable law. Retention periods depend on the specific legal or regulatory duty. If information must be retained, we will explain what is kept, why it is retained and the relevant retention period in our response.

Complete the fields below. When you submit, your email app will open with the request ready for you to review and send to ServPatch.

Your request is not sent until you review it and select Send in your email app.

Important

Deleting a ServPatch account removes access to the app. It does not automatically delete care records controlled by your employer or care-provider organisation.
