Incidents & body maps

When something happens, record it clearly.

A guided, step-by-step workflow helps carers capture what happened, mark any injury on a body map and keep the report connected to the person, ready for a manager to review.

ServPatch incident report at step 3 of 5, showing a front and back body map with mild, moderate and severe injury markers.

Incidents & body maps at a glance

What it does
Guides staff through an incident report, with an optional body map to mark injury location and severity, linked to the person's record.
Who it helps
Carers reporting at the point of care, and managers who need a clear, reviewable account.
The problem it solves
Incidents written up later, on paper or from memory, lose detail and are hard to review together.
How it fits
Incidents sit in the person's incident history, and Patch can summarise recorded incidents for a team review.

How it works

Three steps, inside the working day.

    Start the report

Open the person's profile and choose Report, or start from the incident workflow.

  2. 02

    Capture the detail

Work through each step, adding a body map where an injury needs marking.

  3. 03

    Review and follow up

Managers review the report, and can ask Patch to summarise incidents for a team discussion.

Questions

Good to know.

Is the body map required for every incident?

No. Body map marks are optional, for incidents where an injury needs to be shown clearly. Front and back are captured separately.

Does Patch decide what happens after an incident?

No. Patch can summarise what has been recorded. Follow-up, escalation and care decisions stay with your team and your procedures.
